Our Work In Sierra Leone:

Partners In Health (PIH) began supporting the government of Sierra Leone in response to the Ebola epidemic in October 2014. Recognizing the large need for continued investments in the health system after Ebola, PIH is committed to long-term health system strengthening in support of the Ministry of Health (MOH), guided by the same principles as our emergency response and our work around the world: public sector accompaniment, integrated and comprehensive services, social justice, nimbleness and flexibility, partnership, community ownership, and evidence-based interventions. In the aftermath of the epidemic, PIH is committed to supporting the delivery of comprehensive health services in Sierra Leone, from the community level to the health center and to referral hospitals.  

Position Overview
Under the supervision of the Primary Care Director, the Senior Primary Health Care Specialist will work closely with both the Community-Based Program manager and the Primary Health Care Clinical Manager to implement all of PIH’s primary health care activities in Sierra Leone. He/she will support both managers to develop and implement strategies; improve program integration to improve the links between CBP, the clinical programs and other PIH departments; conduct extensive reviews of existing national policies around CBP and primary care; work closely with MOHS and partners to ensure that PIH as a global leader in primary care program implementation and practices is well-positioned to influence those policies while complying to government regulations. The Snr. PHC Specialist will collaborate closely with other key stakeholders such as NGOs working in the area, community leaders, informal providers to ensure the smooth operations and continued development of an integrated CBP and primary care. The successful candidate will work closely with the primary care Director to develop tools and processes to facilitate efficient management practices, support with fundraising, effective budget management as directed by the PHC director.

 

Qualified candidate will have experience working to build progressive, paid community health worker models that understand the socio-economic drivers of poor health. He/she will also have experience managing a team of professionals in non-profit or governmental organizations in a developing world setting. It is imperative that the candidate has strong interpersonal skills, coordinating with a host of managers and directors across teams that he/she does not directly manage. Humility and a commitment to the accompaniment of colleagues and patients is paramount. This is a position that relies on technical skills and expertise, but also heavily on building interpersonal relationship that will facilitate the necessary level of interaction to successfully build capacity of an already strong and growing Sierra Leonean-led team. 

Essential Duties and Responsibilities:
Specific Responsibilities:

PIH-SL CBP management and Leadership 

  • Supporting the CBP Manager in strategic planning and budgeting for PIH’s CHW program. This includes identifying and helping the development of interventions that will support the CBP strategies, and also support fundraising opportunities that can help to expand PIH SL’s community health programs while preserving the ideals and core functionality of the program. By doing so, the CBP associate will work closely with the CBP manager and the Director of PHC to form linkages with other departments by working collaboratively with other program leads in order to align the CBP strategy with the overall clinical plans and strategies. 
  • Works with CBP Manager to determine which additional standing meetings are required, if any, and then work across teams to ensure those meetings are scheduled, coordinated, attended, and action steps are documented, disseminated and followed up on. 
  • Coordinates closely with other organizations and entities working on CBP program on potential joint effort to strengthen our collaboration with the aim of improving the health and wellness of the vulnerable people we serve. 
  • Support the CBP Manager to ensure that PIH SL’s community programs align with PIH SL’s overall strategies and growth, with a particular focus on ensuring that clinical and community programs interrelate to support patients throughout their continuum of care.
  • Work with the CBP Manager to ensure that the CBP is well aligned and integrated into other specific CBP component of other programs such as the national HIV and TB programs through the network community health workers for those programs. This should also include working on strengthening the health information system for CBP including the reporting of activities to the DHMT and the national programs. 
  • Support the plan to implement the CHW household model by conducting a review of best practices of this model at OnePIH and from other successful implementation with other organizations. Work with the CBP Manager and the Director of PHC to help with the strategic planning and the effective implementation of this CHW household model. Work also with both the CBP and the clinical team to ensure that this household model is designed with the aim to effectively support our key clinical programs such as the HIV/TB, NCD, maternal and child health etc. through the enhancement of household screening, referral system and effective follow-up at community level.
  • Support the CBP manager in collaboration with the clinical and the PHU expansion teams to develop a strategy for general social support, especially nutritional support for vulnerable patients and to study the impact of such support. 
  • Support the CHW and the Social Protection Managers to ensure financial compliance of all PIH CHW program activities at the national and district levels  

Accompaniment and technical support to CBP team

  • Work with CBP Manager to assess the capacity for all CBP Staff, develop and implement training and skills development activities focusing on specific gaps identified. At least 25% of the CBP Associate’s time is expected to be focused on direct skills building sessions with day-to-day mentorship and support to the CBP Manager and the other CBP program leads. Skills building should particularly focus on building partnerships, program development, project management and monitoring, work documentation, strategic decision making, etc.
  • Provide supportive and technical supervision to CHW Manager, Social Protection manager, ANP and social protection officers, ensuring strong collaboration and integration across diverse community health teams. 
  • In collaboration with CBP manager and other program managers such as the HIV/TB program manager, to develop and implement new training programs for CHWs focusing on community-based prevention and case detection of key illnesses such as HIV, TB, mental disorder, etc.
  • Conduct a thorough review of CBPs implemented by other PIH country sites or by other organizations to support the CBP Manager in considering lessons learned and successful strategies from those other CBPs to be considered for the strategic growth of PIH-SL CBP. 
  • To develop a close working relationship with the PIH SL clinical team in order to ensure that program growth and strategic changes are in line with patient needs, as well as with DHMT and broader MOHS to ensure that program growth and changes align with the Sierra Leone government policies and strategies.
  • Work with CBP Managers and M&E team to understand current key performance indicators, and identify with that team additional areas where we can better track our impact. Particular attention should be spent on how to link community and clinic data and how to distill metrics into tangible actions that need to be taken by the CHW team.
  • Support technically the CBP program for an effective implementation of mobile data collection system to be developed in collaboration with CBP, M&E and e-Health teams in order to strengthen CBP overall information system management, reporting and data use for improving patient care and inform programmatic decisions.
  • Geographic Information System (GIS) being a growing area of interest to further support implementation of CBP type activities, the CBP associate is expected to play a significant role in bringing this expertise into the scope of work of the CBP activities. 
  • Assist programs in preparing, writing and editing reports; help prepare abstracts and manuscript submissions to professional journals.

PHU Expansion:

  • Support the clinical manager in facilitating monthly meetings and participate in PHU expansion meetings
  • Engage with the communication team to share updates of progresses being made with regards to the PHU expansion and mentorship project (Pictures, reports, etc)
  • Work with Billy to support TBA supervision community visits and trainings 
  • Support Cooper, Aliyu and DHS1 in PHU expansion trainings, workshops, report writings and activity planning
  • Serve as focal point and liaise with infrastructure, operations, RMNCH, and primary care teams on actions from monthly meetings
  • Work with P&P team in the report writing for PHU expansion supporting grants 

PHU Mentorship program

  • Assist in the coordination and follow-up meetings on PHU mentorship (e.g. with M&E team, training meeting, etc) 
  • Support monthly PHU expansion monthly meetings
  • Track and document all mentorship activities and archive on share point for donor reporting purposes

NCD Expansion, Support and Research

  • Assist in the planning and implementation phase of the NCD expansion to the PHUs. 
  • Support the integration of the NCD into the House-hold model. 
  • Support the PEN program in generating evidence through the NCD implementation at the primary car level.

Education and Work Experience Requirements:
  • Minimum Bachelor’s Degree in public health, Administrative programs or social science  
  • At least 2 years of experience managing and implementing primary/community health programs in resource – limited settings.  
  • Strong grasp of writing and spoken English. Fluency in Krio will be an advantage.
  • Flexible, adaptable, and able to multi-task and delegate effectively.
  • Experience with MEQ, reporting and process improvement systems
  • Excellent written and verbal skills 
  • Strong grasp of Microsoft, Word, Excel
  • Situational awareness, and willingness to support as needed.
  • Skills and ability to work flexibly and responsively within a team. 
  • Able to work independently without a lot of direction and supervision.
  • Understanding of the Sierra Leone healthcare, community, and cultural systems. 
  • A commitment to social justice.
  • Care and compassion for patients and ability to advocate appropriately on their behalf.
  • Schedule available and flexible to occasionally include odd hours and weekends if the situation arises.
